研究在调查杭州市健康服务相关临终关怀认知与态度的基础上,分析影响杭州市健康服务相关临终关怀认知与态度的因素,并提出了具有针对性的意见与建议
摘 要:目的 通过调查杭州市健康服务相关人员对临终关怀认知与态度情况,分析影响健康服务人员对临终关怀认知、态度的因素,提出提升杭州市健康服务相关人员对临终关怀认知与态度状况的相关策略,为杭州市临终关怀事业发展提供相关理论参考。方法 采用自制的“健康服务相关人员临终关怀认知情况”调查问卷,对杭州市不同机构的450名健康服务相关人员进行调查。对分类变量用频数和构成比进行描述;对正态分布的连续性变量用平均值和标准差描述;采用单因素方差分析不同人口学特征及临终关怀的认知对临终关怀态度的差异;用Pearson相关分析临终关怀知识以及态度各维度之间的关系以及密切程度;通过多重线性回归分析临终关怀知识的各维度对临终关怀态度各维度得分的影响,进而得出影响临终关怀态度的因素。结果 临终关怀知识的两个维度中临终关怀基本知识得分为(0.76±0.26),疼痛控制知识得分为(0.46±0.34);临终关怀各维度得分从高到低依次为专业职责与角色(3.75±0.70)、临终关怀的有效性(3.28±0.73)、伦理道德(3.22±0.67)和医患沟通(2.77±0.72)。控制人口学因素后,临终关怀相关知识(标准系数=0.238,P<0.001)对伦理道德的影响显著;临终关怀相关知识(标准系数=0.196,P =0.022)对医患沟通的影响显著;临终关怀相关知识(标准系数=0.408,P <0.001)对临终关怀态度影响显著。结论 当前杭州市健康服务相关人员临终关怀基本知识掌握较好,但缺乏疼痛控制方面的知识;临终关怀知识水平越高,临终关怀态度越积极。

Abstract:Objective: To investigate of Hangzhou health services related personnel of hospice care cognition and attitude, Analysis the influence is healthy service personnel of hospice care the influence factors of cognition, attitude, upgrading Hangzh-ou health services for relevant personnel of hospice care cognition and attitude of relevant policies, provide the theoretical reference to hospice career development in China. Methods: A self-made “Cognitive Situation of Hospice Care for Health Service-Related Personnel” questionnaire was used to survey 450 health service-related people from different institutions in Hangzhou. Using frequency and proportions to describe categorical variables; using mean and standard deviation to describe normal continuous variable; Differences in mortal attitudes between different demographic characteristics and hospice care knowledge using one-way ANOVA; Pearson correlation was used to analyze the relationship between hospice care knowledge and attitudes in each dimension. Through multiple linear regression analysis of the end-of-life cognition and the impact of each dimension on the scores of the hospice attitude in each dimension, the impact of end-of-life care attitudes was then derived. Results: The hospice care basic knowledge scores were (0.76±0.26) and pain control knowledge scores (0.46±0.34) in two dimensions of hospice care knowledge; hospice care dimensions scored from high to low were professional responsibilities and roles (3.75±0.70) ) The effectiveness of hospice care (3.28±0.73), ethics (3.22±0.67) and doctor-patient communication (2.77±0.72). After controlling for demographic factors, the knowledge of hospice care (standard twist coefficien=0.238, P<0.001) had a significant effect on ethical morality; hospice care-related knowledge (standard coefficient of fit=0.196, P=0.022) had a significant impact on doctor-patient communication. The knowledge of hospice care (standard twist coefficient=0.018, P<0.001) had a significant effect on hospice attitude. Conclusion: Currently, the basic knowledge of hospice care of health service-related personnel in Hangzhou is well mastered, but there is a lack of knowledge about pain control; the higher the level of hospice care knowledge, the more positive the hospice care attitude is.
